New Patient Registration

If you wish to register with the practice you must first contact the surgery to check you live within the practice area.  If so you will need to pop into the surgery to collect the registration forms or a registration form can be downloaded from this website. 

We ask for ID such as a passport or driving licence and also a utility bill as proof of address.  Please contact your current surgery to obtain a month’s supply of medication as it does take a few weeks for your medical notes to reach your new surgery.   If you have entered the UK and are now living in England, you will need to advise us of the date you came into the UK.  Your registration cannot be completed without this information.

Change of Address/Details

Please advise the surgery as soon as possible if you have moved address, changed telephone or mobiles numbers or got married or changed surname.  You will need to collect a form from reception or this can be downloaded from the website.
